COMPLIMENTARY SPORT SCREENINGS FOR HOOVER ATHLETES

MOBILE CLINIC FLYER 1 and MOBILE CLINIC FLYER 2 – CCHS will be visiting Hoover June 7^th, 10^th, 17^th, 21^st and July 1^st, 2^nd, and 3^rd to provide complimentary sport screenings for our athletes.  You must register ahead of time by using the contact information provided ont the flyer.  Be sure to bring a completed parent/guardian consent form if arriving without parent/guardian, along with the GUSD sports physical form.  

Note: GUSD is offering a fantastic opportunity for students, especially athletes, to receive sports physicals through CCHC's mobile screening van. These physicals are at no cost to those without insurance or on Medi-Cal. For those with a private insurance who would still like to be seen with the mobile clinic there will be a standard fee of $40 for the visit. EXER URGENT CARE – Exer Urgent Care has partnered with Hoover HS to provide our athletes with complimentary sport screenings at over 80 loccations in So. Cal.  Be sure to book an appointment online, select Hoover High School, and take the GUSD sports physical form with you.
